We
participate with most of the insurance plans available in this area.
It is your responsibility to provide us with information about your
insurance coverage. Any time you have a change in coverage, contact
our office immediately. We will ask to see your insurance cards
each time you visit the office. Please have them available so we
can make a copy for your file.
If your insurance requires a referral from your primary
care physician, it is your responsibility to obtain one prior to
your office visit. Patients without a valid referral will be asked
to sign a waiver of responsibility until one can be obtained.
If your insurance requires you to pay a copay, this
will be collected before you see the physician. Copayments can be
made by cash or check.
We file all claims to all of your insurance plans
we have listed. Statements are mailed the third week of each month
whether you owe any balance or not. It will note on the statement
your insurance was filed and any amount due from the patient will
be listed on the top portion of the statement in the "Patient Owes"
column. If you notice your insurance has not paid by your second
statement, please contact your insurance company for status. If

If you experience a financial problem, we want to
work with you to find a solution. Please contact our billing office
to make arrangements for a plan that will help you pay your balance
and maintain your credit rating.
